The purpose of this directory is to provide comprehensive information about the local cancer services available to the population of Calderdale & Huddersfield. It has been produced to build on existing communication links between primary and secondary care

It includes contact details for key personnel in the delivery of cancer care, information on how to make a referral and details about local support and information services.

MODERN SLAVERY ACT 2015 The Modern Slavery Act 2015 is designed to consolidate various offences relating to human trafficking and slavery. The provisions in the act create a requirement for an annual statement to be prepared that demonstrates transparency in supply chains. In line with all businesses with a turnover greater than £36 million per annum, the NHS is also obliged to comply with the Act.

The Friends and Family Test

The Friends and Family Test

On the 25th May 2012 the Prime Minister announced the introduction of the Friends and Family Test to improve patient care and identify the best performing hospitals in England.

How the test works

Every patient who attends Calderdale and Huddersfield Trust is given the opportunity to provide feedback about their care and treatment.

Patients are asked to complete a “Friends and Family Test” questionnaire before going home, or they are contacted following their attendance by a 'free of charge' text/voice messaging service within 48 hours.

Patients will be asked the following questions and are invited to tell us why they made their selection.

Why are we doing this?

The test enables the views of patients and their families to be heard and provides a platform to shape and deliver better services. It means that all our staff from 'Board to Ward' will be informed and empowered to tackle areas of weak performance and to celebrate and build on what is working well, using the results from this test alongside other sources of patient feedback.

We continuously gather patients’ comments and analyse them to see where we can make improvements or explore suggestions further. If comments are complimentary we will, of course, pass on this praise to the staff involved.

Our Matrons and Ward Managers have access to all the feedback their ward or department receives so that they can discuss this with the rest of their team and take any appropriate action.

Publication of results

The results of the Friends and Family Test are published by NHS England on a monthly basis. The latest available results for Calderdale and Huddersfield NHS Trust for each area can be found below. Please also see below for examples of patients’ comments:
