Virtual Hub
The Virtual Hub incorporates both the Virtual Ward and the coordination of OPAT (Outpatient Parenteral Antimicrobial Therapy) services for Calderdale and Huddersfield. Our primary aim is to prevent unnecessary hospital admissions and support timely hospital discharge.
The Virtual ward - Patients (registered with a Calderdale GP) who are identified as being at increased risk of readmission into hospital are admitted onto the Virtual Ward caseload following their discharge from hospital. These patients are then pro-actively case managed in an attempt to reduce the risk of admission by working in partnership with the patient, General Practitioner (GP), Social Services, Secondary Care Services and Voluntary Organisations to ensure the patient receives the right health care at the right time in the right place. In an emergency or out of hours, patients should continue to access all other services in the usual way. The service aim is to coordinate and optimise the social, medical and psychological health of the patients admitted to the caseload through a collaborative team approach.
OPAT - Outpatient Parenteral Antimicrobial Therapy (OPAT) is a service that allows patients to have IV antibiotics administered either in their own home or as a ward attender, rather than having to stay in hospital as an inpatient. OPAT leads to improved patient choice and patient experience and provides an alternative to inpatient treatment, therefore freeing up hospital beds, reducing risk of Healthcare Associated Infections & saving money.
