Cardiac rehabilitation services

We are a multidisciplinary team of nurses, Physiotherapist, Exercise physiologist, rehabilitation assistants and clerical staff, based at Dean Clough Mills in Halifax, but working across both Calderdale and Huddersfield hospitals and in the community in Calderdale.

The qualified team members are specialised in caring for patients following a cardiac event, they have undertaken specialised training courses for the safe exercise of cardiac patients in accordance with BACPR (British Association for Cardiovascular Prevention and Rehabilitation).

Cardiac rehabilitation aims to support people to improve their physical health and quality of life, also to equip and support people to develop the necessary skills to successfully manage their condition.

The team see patients both in their home in clinics and offer a menu of outpatient programmes. Including gym based, aerobic, tai chi and chair-based activities. Along with information sessions, covering healthy lifestyles and conditions at a variety of times throughout the week.

The programme also includes stress management and a healthy eating group.

Heart Failure Service

We are a team of Specialist Heart Failure Nurses, based at Allan House but working across both Calderdale and Huddersfield hospitals and in the community in Calderdale.

The qualified team are specialised in caring for patients with Heart Failure. The aim of team is to monitor signs of Heart Failure, titrate medication, provide supportive literature, and educate regarding heart failure diagnosis and treatment.

Patient education focusing on lifestyle changes, symptom management and promoting self-care and patient monitoring of signs of deterioration and what action to take.
