How to refer community deaths for scrutiny

July 2023 update:  Pilot testing of referrals via the new SystmOne unit is only available through collaborative working with four GP surgeries in the week commencing 3rd July 2023. This is being expanded to an additional eleven practices on Monday 17th July 2023.

August 2023 update: Additional nine GP surgeries onboarded. Since Tuesday 22nd August 2023: the ME referral form is available on Ardens eReferral. October 2023 update: Additional thirty one GP surgeries onbaorded. Kirkwood Hospice deaths now also scrutinised. And we are currently developping systems to be able to serve the ten EMIS practices in the area.

November 2023 update: Roll out is continuing to two EMIS GP practices. The Forgot-Me-Not Children's Hospice also joined this month.

The service will provide ME scrutiny to all GP surgeries & hospices in Calderdale & Kirklees by April 2024 as the requirement becomes statutory [see Coroners and Justice Act 2009 (legislation.gov.uk)]

After pilot testing, the link for EMIS GP surgeries, Hospices that do not use SystmOne or another other community healthcare provider in Calderdale or Kirklees to refer community deaths for ME scrutiny will be published here. Watch this space. Expected launch Summer 2023.
