Our Community Healthcare Self-Management Champions empowering patients to better manage their own conditions

This June as part of an awareness building campaign, our 10 Community Healthcare self-management champions will be sharing information and guidance aimed at empowering patients to help manage their own conditions.

There are currently 10 champions who have all volunteered to do the role alongside their day-to-day practice.

Occupational Therapy Clinical Educator, Jessica Loxam, said: “Our champions promote self-management, which is a technique used when delivering patient care. It aims to encourage, support and empower patients to manage their own health conditions in the best way for them.

“Self-management encourages patients to set their own goals, as evidence has proven that this technique supports better patient outcomes and reduces the reliance on healthcare services.”

The champion’s pledge reads:

"As Self-Management Champions we pledge to keep the patient at the centre of our care through the promotion of self-management skills within the community division as a must do, with an aim to empower people to take control of their acute or chronic health condition."

Our champions meet every eight weeks to work towards goals and projects. Their first project is to introduce the concept of self-management, and the new champion roles, to the Community Healthcare Division.

Jessica added: "We have all had training and have started to implement different techniques within our own practice and can see huge benefits to our patients. We therefore feel it's important to find ways to best share this knowledge and these skills. We are hoping the role of Self-Management Champions will help us to come together to share skills and resources."
