How to refer community deaths for scrutiny
July 2023 update: Pilot testing of referrals via the new SystmOne unit is only available through collaborative working with four GP surgeries in the week commencing 3rd July 2023. This is being expanded to an additional eleven practices on Monday 17th July 2023.
August 2023 update: Additional nine GP surgeries onboarded. Since Tuesday 22nd August 2023: the ME referral form is available on Ardens eReferral. Pending software testing, user engagement & iterative improvements, the service will add further guidance and ressources for GP & hospice users here.
The service aims to provide ME scrutiny to all GP surgeries & hospices in Calderdale & Kirklees by October 2023 with a monthly roll-out to 12-15 providers each month.

After pilot testing, the link for EMIS GP surgeries, Hospices that do not use SystmOne or another other community healthcare provider in Calderdale or Kirklees to refer community deaths for ME scrutiny will be published here. Watch this space. Expected launch Summer 2023.
