Children’s Community Nursing Team

The CCN service is a Paediatric consultant led service based within CHFT. We are a team of highly trained RSCN’s all of whom have a wealth of experience some of which hold the Children’s Community specialist degree & also some very experienced HCA’s. Based within the team are the children’s special interest nurses for Oncology, Cystic Fibrosis and Epilepsy.

The CCN team see children and young people from new-born to their 16th birthday.  The team also see young people over the age of 16 years, if they are under the direct care of a Consultant Paediatrician.
Each child/family is allocated an RSCN Children’s Community nurse who oversees the planning & delivery of care and its evaluation. Care is, however, delivered on a team approach by all team members, including our experienced Band 3 Health Care Assistants.

All our qualified staff are registered and monitored by the NMC (Nursing & Midwifery Council) and have up to date enhanced DBS checks as do the HCA’s on the team.

The CCNT work closely with children, parents and all other relevant professionals involved with the child ensuring care is carried out in the best way possible.
