Making a referral

Who can refer?

  • Acute paediatric ward Medical team and ward staff at CHFT.
  • Accident & Emergency department CHFT
  • Paediatric tertiary centres – from all paediatric specialities.
  • Day case surgery units CHFT
  • Consultant Dermatologists CHFT
  • Child development Service CHFT

Children must live within the Huddersfield /Calderdale boundary and they must have a Huddersfield 
or Calderdale GP to have access to the CCNT service.
