Healthcare Travel Scheme

The Healthcare Travel Scheme is a department of Health scheme which alows patients on a low income or who receive specific benefits to claim the travel costs to hospital.  Qualifying criteria and procedure is explained in the leaflet please click the link to access the information.

CALDERDALE

Calderdale community transport service

https://www.ctcalderdale.co.uk/  - can register online.

There is an admin fee of £5 to join.

To help cover their costs there is a £2.50 minimum charge per trip and a 50p mile charge after 5 miles.

Please note this mileage cost applies for the drivers to and from their homes.

They also offer a service called home from Hospital where they will take anyone home from hospital.

Referral process:

  • Has to be a part of the medical team that call 0300 020 0213 
  • when you know what time the patient will be ready to leave
  • There maybe a waiting time anything up to two hours - same as transport 
  • no carry chairs or stretchers

 

 

KIRKLEES

TIMEtogether, formerly known as The Denby Dale Centre, is a Kirklees-based charity that provides various community transport services to connect people, reduce isolation, and improve quality of life. 

They offer a range of options, including volunteer car services, Shopper Bus, Social Bus, group minibus hire, and even self-drive minibus hire. Their services operate throughout the Kirklees area, including Denby Dale, Kirkburton, and other locations. 

TIMEtogether's Transport Services:

  • Volunteer Car Service: Volunteers drive individuals to essential appointments like doctor visits, hospital appointments, and vaccinations, as well as to social activities and shopping.  Click here for a poster with more information.
  • Shopper Bus: Provides transportation for residents who need help with shopping. 
  • Social Bus: Offers transportation to social and community events. 
  • Group Minibus Hire: Accessible minibuses can be hired for community groups, social groups, or schools for not-for-profit trips. 
  • Self-Drive Minibus Hire: Provides the option to rent a minibus for personal use. 
  • Other Services: They also offer services like Ukraine Transport, Vaccination Transport, and may provide other customized solutions.

Seamless Home from Hospital Service

This is a free transport and support service for elderly and vulnerable patients in Calderdale and parts of Kirklees.

How to book the service

The ‘Home from Hospital’ service must be booked through the Hospital ward staff in Calderdale or Kirklees and then they will contact our Home from Hospital coordinator.
