Specialist palliative care services

Specialist Palliative Care

Our specialist palliative care teams support people living with a serious or life‑limiting illness. We are here to help manage difficult symptoms, offer emotional and practical support, and guide you and your loved ones through important decisions about your care.

How we support you

Our teams work closely with hospital staff, GPs and community services to make sure you receive the right care at the right time. Support may include:

  • Help managing symptoms such as pain, nausea or breathlessness
  • Emotional support for you and those close to you
  • Advice about future treatment options and your preferred place of care
  • Linking you with other services at home, in hospital, a care home or a hospice

We can offer advice to healthcare professionals involved in your care, or we may meet with you directly if needed.

Who we support

We accept referrals for any adult patient in CHFT hospitals or in the Calderdale community who may benefit from specialist palliative care. Referrals are usually made by a healthcare professional.

If you wish to have most of your care at home, we work with community teams including our colleagues at the Kirkwood Community Palliative Care Team to help make this possible.

Hospice support

If a hospice stay might be helpful, our specialist palliative care teams will carry out an assessment and coordinate the referral.
In the Kirklees community, this assessment is provided by our partners at Locala.

Our local hospices are:

  • Overgate Hospice, Elland – for patients with a Calderdale GP
  • The Kirkwood, Dalton – for patients with a Kirklees GP

Chaplaincy Team

When you or someone close to you is unwell, it’s completely natural to look for comfort, strength or simply someone to listen. Our multi‑faith Chaplaincy Team is here for everyone  whether you follow a particular faith, have your own spiritual beliefs, or are not religious at all.

The chaplains can offer:

  • A quiet space to talk
  • Emotional and pastoral support
  • Prayer or reflection, if this is something you would find helpful

Interpreting Services

Conversations about palliative care can feel overwhelming, and it’s important that you and your loved ones can understand everything clearly. If English is not your first language, or if you use British Sign Language, we can arrange an interpreter to support you during discussions and appointments. Just let a member of the team know.
