Who we are

BLOSM supports vulnerable people who come to Accident & Emergency (A&E) at Calderdale Royal Hospital or Huddersfield Royal Infirmary.  

Our multidisciplinary team supports people aged 11 and over, both in the hospital and in the community. We help people by understanding their unique needs and helping them find appropriate community services which means they don’t need to come to an emergency department for support.

Aim

Our aim is to reduce people coming to A&E when they don’t need to, address complex social and health needs and support patients in achieving long-term stability and wellbeing.

We provide timely, person-centred support to patients attending emergency care. We work to identify and support people who frequently attend emergency departments, to make sure they are offered the right help at the right time.

Our Youth Navigation Service supports young people aged 11–25 both in hospital and in the community, while our Substance Care Team provides focused support for those experiencing issues with drugs or alcohol. We are committed to putting people in touch with the right services in the community to help with recovery.

Branches of BLOSM

There are four key branches within BLOSM:

1. Youth Navigation Service

Supporting young people aged 11–25 both in hospital and community settings, including follow-up and collaborative work with local and national partners.

2. Substance Care Team

Offering support to patients with drug and alcohol concerns, including referrals to specialist services and harm-reduction advice.

3. High Intensity User Group (HIUG)

Working with individuals who frequently attend A&E, helping reduce visits through tailored, multi-agency care plans.

4. Staff Development and Training

Promoting trauma-informed care, providing education on how to best support complex and vulnerable patients in the emergency setting.
